Care 4 You care agency registered manager carries out assessments on proposed service users. This involves the service users, their family, and their trained caregivers. When this is done, a transition plan is prepared which highlights urgent needs, risks, past issues, good procedures that has worked for the service user in the past and the handling of care to local careers.
Having in mind client needs, the service users will be gradually introduced to the service and then transfer to local care will be completed. This includes Gp services and domestic health services where needed. Care 4 You care agency staff will conclude a plan with the service users providing the best support throughout the process.
We are devoted to measuring results, we make the service users aware of their improvements and help them focus on that which is important. These measures are vital when developing their personal care plan.
